For prescription refills, contact your pharmacy. If no refills remain, the pharmacy will contact your Intermountain care team. Effective August 1, 2021, only electronic refill requests will be accepted. We will no longer accept requests via fax. To help avoid delay with your refill request, please relay this information to your pharmacy.

Please note that many of our clinics are closed on the following holidays: New Year’s Day, Martin Luther King, Jr. Day, Memorial Day, Independence Day, Labor Day, Thanksgiving and the Day After Thanksgiving, Christmas Eve and Christmas Day.
